According to multiple reports, the Eagles fear that DE Brandon Graham tore his left Achilles' tendon in Sunday's loss to the 49ers. We'll see about the official diagnosis, but such "fears" tend to be realized.

Steelers WR Diontae Johnson injured his right knee on the final play of Sunday's game vs. the Raiders. He needed help from trainers to get off the field. We'll look for an update.

49ers RB JaMycal Hasty injured an ankle in the 2nd half of Sunday's game against the Eagles and is officially questionable to return. RB Trey Sermon, meanwhile, is being evaluated for a concussion after losing a fumble on his 1st carry of the season. RB Elijah Mitchell at least returned for San Francisco after hurting his shoulder.

Colts QB Carson Wentz was knocked out in the 4th quarter of Sunday's game vs. the Rams with a right foot or ankle injury. QB Jacob Eason entered and threw an INT on his 2nd pass. We'll update Wentz's status when we know more.

Rams RB Darrell Henderson suffered a rib injury in the 2nd half of Sunday's game vs. the Colts and is officially questionable to return. Stay tuned for updates.

Titans LT Taylor Lewan suffered a leg injury in pregame warmups-ups and is inactive vs. Seattle

49ers RB Elijah Mitchell left Sunday’s game with a shoulder injury. Beat writers observed Mitchell getting his shoulder looked at by team trainers. With only 8 minutes remaining, we’ll see if he returns to the field.

UPDATE: Mitchell returned late in the 4th.

Seahawks CB D.J. Reed (calf) is active for today’s game against the Titans. Use him as you had planned in IDP lineups.

Cowboys S Damontae Kazee (thigh) is active for today’s game against the Chargers.

Titans LB Jayon Brown (hamstring) is inactive for today’s game against the Seahawks. David Long figures to start in his place next to Rashaan Evans. OLB Bud Dupree (knee) is active but a low-end IDP option at best.

Titans TE Anthony Firkser (knee) is inactive for today’s game against the Seahawks. That leaves Geoff Swaim and MyCole Pruitt as Tennessee’s top TEs.

Seahawks WR D’Wayne Eskridge (concussion) is inactive for today’s game against the Titans. Freddie Swain should again operate as Seattle’s #3 WR behind D.K. Metcalf and Tyler Lockett.

Seahawks RB Rashaad Penny (calf) is inactive for today’s game against the Titans. His absence should mean a few extra touches for RB Chris Carson. RBs DeeJay Dallas, Travis Homer and Alex Collins are active behind Carson.

Texans QB Tyrod Taylor hurt his hamstring early in the 3rd quarter of Sunday's game vs. the Browns and was quickly ruled out for the day. Rookie QB Davis Mills will take over for Houston.

Buccaneers CB Carlton Davis (hamstring) is active for today’s game against the Falcons. He’ll carry some elevated risk but also upside vs. Calvin Ridley

Vikings LB Eric Kendricks (quad) is active for today’s game against the Cardinals. Use him as you normally would in IDP lineups.

Eagles DE Brandon Graham left in the 2nd quarter of Sunday’s game against the 49ers with a left leg injury. He left the field unable to put any weight on it and rode a cart to the locker room. We’ll watch for updates.

Dolphins QB Tua Tagovailoa has been diagnosed with bruised ribs, NFL Network's Tom Pelissero reports. X-rays came back negative, and Tua will get an MRI on Monday "He's in a lot of pain," Pelissero adds.

Steelers OLB T.J. Watt left in the 1st half of Sunday’s game against the Raiders with a groin injury. He’s questionable to return.

Texans WR Nico Collins is questionable to return to Sunday’s game with a shoulder injury. We’ll let you know if he returns to the field. Before departing, the rookie caught a 32-yard pass.

UPDATE: Collins was ruled out in the 2nd half.
